CMS is pleased to announce that the 2018 Quality Payment Program self-nomination period for vendors interested in participating as a Qualified Registry or a Qualified Clinical Data Registry (QCDR) will begin on September 1, 2017 and will close at 5:00 PM (Eastern Time) on November 1, 2017. Please submit your completed self-nomination form prior to the close of the self-nomination period to be considered; late submissions will not be accepted.

If you have submitted your completed 2018 self-nomination form to CMS prior to the close of the self-nomination period, and your application is ready for review, please notify us by email. Please email QCDRVendorSupport@gdit.com if you have submitted a QCDR self-nomination form for consideration, or RegistryVendorSupport@gdit.com if you have submitted a qualified registry self-nomination form. Please make sure to include your 2018 self-nomination form JIRA ticket number in the subject of your e-mail.
